Drama Camp Liability Release Form

Dear Parents and Legal Guardians:

Pinewood Players, Inc. takes great pride in the quality of our supervision in our drama camp and we do everything in our power to create a safe and secure environment for our campers and our volunteers. However, we are still required to obtain a legal waiver/release from all families with children attending our drama camps and participating in performances.

The liability waiver and release below are required for every child enrolled in Pinewood Players Drama Camp. We must have a copy of this waiver signed by a parent or legal guardian on file for each child prior to their participationin the camp.

Your child’s health, welfare and enjoyment while attending our camp are our primary concerns. Please take a moment to review and sign the liability release below. Again, we must have a separate form on file for every child attending the camp. We are looking forward to having your camper(s) participate in our camp this year.

As a parent/legal guardian of the child identified below who is partcipaing in the Pinewood Players Drama Camp from June 29th through July 12th (Kidz Kamp) or June 22nd through July 12th (Youth Camp), I agree that I will not institute any legal acton on behalf of myself or my child, personal representaives, assigns, heirs or next of kin for damages, claims, injury or other liability against Pinewood Players, Inc., or any member of Pinewood Players, Inc., its Board of Directors, employees, or agents. Further, I agree that I will not institute any legal action on behalf of myself or my child's personal representatives, assigns, heirs, or next of kin for damages, claims, injury, or other liability against Pinewood Country Club, Inc., any member of the Pinewood Country Club, Inc., its Board of Directors, agents, or employees. This release shall apply to all sponsors, directors, producers, their agents, representatives, employees, or successors; and applies to any and all costs or expenses which I may individually or on behalf of my child incur against said individuals arising out of or in any way connected with my child’s participation in the Pinewood Players Drama Camp, including travel to and from this event, and including injuries which may be suffered by my child before, during, or after the event. I understand that this waiver includes any claims based on negligence, action, or inaction by any of the above-named parties.

The undersigned agrees to indemnify, release, defend, and hold the Pinewood Players, Inc., any member of the Pinewood Players, Inc., and its Board of Directors, employers, and agents harmless against any damages, claims, or other liability resulting from the child's participation in activities sponsored by Pinewood Players, Inc. The undersigned further expressly agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the State of Arizona, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

Permission is granted to the Pinewood Players, Inc. to use photographs taken for any legal use, including but not limited to publicity, copyright purposes, illustra4on, adver4sing and web content. Furthermore, it is understood that no royalty, fee, or other compensation shall become payable to us by reason of said use.
